The Home Health Physical Therapist evaluates plans and administers comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician.
Full Time: Monday - Friday day shift availability. Hours are typically from 8:00a.m.-4:30p.m. Hours may be flexible depending on patient needs. Mileage reimbursement offered from visit to visit and paid drive time! Excellent benefits including medical, dental, vision, 401k, PTO, and many more!
Reserve status also available: Day shift hours with flexible scheduling!
Must be a graduate of an accredited physical therapy curriculum/school approved by:
The American Physical Therapy Association or
The Committee on Allied Health Education and Accreditation of the American Medical Association or
The Council on Medical Education of the American Medical Association and the American Physical Therapy Association
Current State of Wisconsin Physical Therapy Licensure
Current CPR card from the American Heart Association
Current BCLS certification.
*Must also have valid state driver’s license and reliable automobile, current automobile insurance and be willing to operate personal car.
