Entry level position; excellent learning opportunity for newly licensed professionals looking to gain clinical experience in an inpatient setting.
Care Coordinators are responsible for providing therapeutic services for patients. This position is responsible for interviewing and developing the psychological history, assessing the therapeutic needs of the individual and family throughout treatment, developing appropriate treatment objectives and summaries, and assessing patients' response to treatment interventions. Care Coordinators work closely with other clinical and program staff in treatment planning and delivery of clinical services, attending all scheduled clinical staff meetings and treatment planning meetings. This position acts as the patients' and families' primary contact during the patient's stay at the facility, as well as their advocate with the treatment team. This position is also a liaison with outside agencies and individuals for the purpose of developing and coordinating discharge planning with the treatment team and family.
Work schedule is five days a week to include evenings and weekends as needed. Care Coordinators are assigned either a Tuesday - Saturday or Sunday - Thursday schedule.
