Physician (Primary Care)
Provides preventative medicine and medical care management for adult patients for acute and chronic medical condition. Responsible for the direct treatment, consultation and/or referral for patients' medical conditions as appropriate Management of patients includes following VA clinical guidelines with documentation in the electronic medical record. Works within a team of various health care staff to direct the resources needed to assist in the management of patients. Physician (Primary Care) duties consist of but are not limited to the following: Provide comprehensive primary care and preventative care to an established panel of primary care patients, under the Patient Aligned Care Team (PACT) model Participate in interdisciplinary team meetings and functions as a team member.
